Schedule an Evaluation
What do I need in order to schedule an appointment?
Physician's orders for physical therapy. If your insurance requires a referral, that needs to be obtained from your PCP prior to your first appointment.
What do I need to bring with me on my first visit?
Your insurance card, script from your doctor for physical therapy, and clothing that will allow access to body part being treated.
What insurances are accepted?
With the exception of medicare and medicaid, we accept all insurances including worker's compensation and motor vehicle cases.
What if I don't have insurance?
We have a self-pay rate for patients that do not have insurance. Please call any of our locations for more information on the rates.
What if a patient is under the age of 18? Do they need to be accompanied by a parent or legal guardian?
A parent/guardian will need to attend only the first visit if the patient is under the age of eighteen to fill out appropriate paperwork.
How long is an average therapy session?
The first visit is an evaluation typically 30 minutes in duration. Follow-up appointments are typically 45-60 minutes in duration.
